PT Home Health (Travel/Contract)

Location: Saint Joseph, Missouri
Specialty: Therapy/Rehabilitation
Contract Length: 13 weeks


Job Details

  • Home Health Physical Therapist role with pending Missouri license approval acceptable.
  • Must have prior Home Health experience (minimum 1 year required).
  • Shift options:
    • 5 days per week, 8 hours per day (8:00 AM – 4:30 PM)
    • Or 4 days per week, 10 hours per day (preferred by traveler)
  • Call required 1 Sunday every 5 weeks.
  • Weekend rotation: every 5th week, take 2 weekdays off and work Saturday and Sunday to perform patient admits.
  • Coverage area includes 13 counties; travel and mileage vary, with patient load adjusted accordingly.
  • Dress code: No printed scrubs; black, grey, or khaki pants with polo or scrub top.

Required Qualifications

  • Valid Physical Therapist license in Missouri (primary source verification required within 30 days of start).
  • Minimum 1 year of Physical Therapist experience within the last year.
  • Minimum 2 years of nursing experience.
  • Experience in a Home Health setting.
  • Basic Life Support (BLS) certification issued by American Heart Association or American Red Cross (if ARC, must state "Basic Life Support").
  • Doctor of Physical Therapy (DPT) degree.

Compliance & Documentation Requirements

  • Background checks within 30 days of start, including:
    • National criminal check
    • State and county searches (10 years of all addresses and aliases)
    • SSN trace with aliases
    • National Sex Offender registry
    • FACIS Level 3
    • EPLS/GSA/SAM
    • HHS/OIG
  • Education verification with official transcript or diploma including graduation date.
  • Missouri EDL Search required within 30 days of start.
  • Driver’s license or state ID required.
  • Employment application, skills checklist, and work history verification required.
  • Two professional references within 3 years of start date.

Health & Safety Requirements

  • Physical exam within the last 12 months (not required annually).
  • Drug screen within 30 days of start (includes cocaine, barbiturates, amphetamines, benzodiazepines, opiates, methaqualone, propoxyphene, methadone, PCP, MDMA/MDA).
  • Fit mask testing completed onsite at orientation.
  • Immunizations/vaccinations or documented immunity/declination required for:
    • Rubeola (Measles) – 2 MMR vaccines, positive titer, or declination
    • Mumps – 2 MMR vaccines, positive titer, or declination
    • Rubella – 2 MMR vaccines, positive titer, or declination
    • Hepatitis B – vaccination series, titer, or declination
    • Varicella – 2 vaccines, positive titer, or declination
  • Negative TB test (PPD, Q-Gold, or T-SPOT) within 1 year of start, updated annually. For positive TB results, chest x-ray within 6 months and annual TB questionnaire required.
  • Seasonal flu vaccination required during flu season or agency-approved medical/religious exemption (declinations not accepted). Mask required if exemption granted.

Additional Notes

  • Locals are accepted at the same rate as travelers.
  • Candidate must review and sign confidentiality agreement, code of conduct, and complete demographic form prior to start.
  • Orientation includes safety manual acknowledgement and agency nurse orientation quiz (minimum 84% passing score required).
  • Pass/fail testing and competency exams may be required as applicable by profession and specialty.
