PT Home Health - Oregon (Travel/Contract)
Location
- City: Seaside, Oregon
- Work area includes Seaside and surrounding areas, with occasional travel radius up to 60 miles (60-90 minutes total drive time per day).
Job Details
- Employment Type: Travel/Contract
- Contract Length: 13 weeks
- Shift: Day shift, 5 days a week, 8 hours per day (8:00 AM - 4:30 PM)
- Productivity: May visit up to 6 patients per day; 30 productivity points standard
- Guaranteed Hours: Facility may call off up to one shift every two weeks
- Weekend Work: Not required
- On-Call: Not required

- Locals accepted within 50-75 mile radius at the same rate

Required Qualifications
- Valid Physical Therapist license in Oregon
- Minimum of 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Basic Life Support (BLS) certification
Background and Compliance Requirements
- Background Check: Must be completed within 90 days of start, covering all locations lived/worked in past 7 years; includes SSN trace, death master index, nationwide checks, motor vehicle record (required for home health), and state-specific abuse/neglect searches as applicable
- Education Verification: Required to verify relevant education credentials
- Drug Screen: 9-panel drug screen within 12 months of start (instant acceptable)
- Physical Exam: Within 1 year of start, signed by MD, NP, PA, or DO, indicating fitness for duty and free of infectious disease
- Immunizations and Health Requirements:
- Tetanus/Diphtheria/Tdap within 10 years (declination accepted)
- Negative TB test or chest X-ray within 12 months (annual updates required)
- Measles, Mumps, Rubella: 2 MMR vaccines or positive titer (declination accepted)
- Varicella: 2 vaccines or positive titer (declination accepted)
- Hepatitis B: Documentation of vaccination series or signed declination
- Seasonal Flu Vaccine: Required during flu season (declination accepted)
- COVID-19 Vaccination: Documentation preferred; declinations not required but unvaccinated must follow masking policies
- Respiratory Safety Clearance: Required annually for respirator use (travel only)
- Fit Mask Testing: Required annually for travel candidates (onsite testing available if needed)
References and Employment History
- Minimum of 2 professional references covering at least 1 year of employment within the last 3 years in a comparable specialty/setting
- Employment application covering at least the last 5 years of employment with month/year format for dates worked
