Description
Case Manager - Hospice Nurse
Location: Grand Junction, Colorado
Employment Type: Contract
Contract Length: 13 weeks
Job Description
- Full-time position with a Monday to Friday schedule, 8am to 5pm.
- Rotating on-call and weekend coverage required occasionally.
- Provide compassionate hospice nursing care in various settings including private homes, nursing homes, group homes, and assisted living facilities.
- Serve as a Primary Registered Nurse with an assigned caseload, leading the interdisciplinary team for those patients.
- Act as a teacher to patients and caregivers, helping them set personal care goals and build confidence in caregiving.
- Manage symptom control and provide generalist care across all diseases, requiring strong assessment and critical thinking skills.
- Flexible daily schedule to support work/life balance.
Required Qualifications
- Valid Registered Nurse (RN) license in the state of Colorado.
- Minimum of 2 years of hospice nursing experience, including experience within the last year.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Additional Information
- Prior travel experience is not required.
- This role involves hospice nursing, focusing on end-of-life care and symptom management.
