RN Case Manager - Home Health
Location: White River Junction, Vermont (also serving surrounding VT and NH areas)
Employment Type: Contract
Contract Length: 13 weeks
Shift: Days
Hours per Week: 36
Job Description
- Provide comprehensive home health nursing care as an RN Case Manager.
- Screen, evaluate, and re-evaluate patients; develop and periodically review care plans in collaboration with physicians, patients, and caregivers.
- Coordinate patient care through interdisciplinary communication and case conferences.
- Monitor patients’ clinical, functional, and emotional status; communicate significant changes to physicians and team members.
- Document evaluations, treatment goals, and objectives.
- Teach patients and caregivers self-care techniques, medication, diet, and provide health counseling as appropriate.
- Work with broad independence while being part of a cohesive patient-care team.
- Maintain a work/life balance with flexible scheduling.
Required Qualifications
- Valid Registered Nurse (RN) license in Vermont
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Minimum 2 years of experience in Home Health nursing, including experience within the last year
- Experience working in a Home Health setting
Additional Information
- Full-time position with day shifts.
- Service area includes multiple towns in Vermont and New Hampshire.
