Job Description
- Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders.
- Serve as Case Manager when acting as the lead discipline on a case.
- Deliver home health and hospice care focused on holistic, community-centered support.
- Support a range of services including health promotion, long-term care, and maternal-and-child health.
Required Qualifications
- Current RN license valid in Vermont.
- BLS/BCLS certification.
