Registered Nurse (Home Health) – Vermont
Job Type
- Contract
- Full-Time
- 36 hours per week
- Day shift: Monday to Friday with weekend rotation approximately every 4th week
Location
- Barre, Vermont
Job Description
- Provide individualized, goal-oriented nursing care using the nursing process in accordance with physician orders.
- Serve as Case Manager when acting as the lead discipline in cases.
- Deliver home health and hospice care focused on holistic, community-centered support.
Required Qualifications
- Valid RN license for Vermont.
- BLS/BCLS certification.
Contract Length
- 12 weeks
