Description
RN Case Manager
Location: Manhattan, Kansas
Employment Type: Long-Term Assignment (13 weeks)
Shift: 08:00 AM – 04:30 PM
Job Summary
Coordinate the interdisciplinary plan of care for patients from admission through discharge within the Case Management department. Responsibilities include:
- Developing and integrating patient/family-centered care plans across the continuum
- Advocating for patient resources and removing barriers
- Monitoring medical necessity and level of care through assessments and evaluations
- Collaborating with supervisors and care transition teams to implement and reevaluate care plans
- Serving as a resource for medical record documentation and level of care recommendations
- Acting as a liaison between patients and post-acute services
- Collaborating with payers to optimize benefits
- Promoting effective and efficient use of clinical resources
Required Qualifications
- Valid RN license in Kansas
- Minimum of 1 year of Case Management experience within the last year
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- Basic Life Support (BLS) certification issued by the American Heart Association
- MOAB Moderate Risk certification
Onboarding and Compliance Requirements
Must be completed prior to or at the start of assignment:
- Professional credentials: Resume/Profile, two professional references, criminal background check, employment verification, education verification, signed assignment agreement, onboarding attestations, and contract worker onboarding
- Certifications: BLS, MOAB Moderate Risk De-Escalation Training
- Health and Immunizations:
- Ascension Color Vision test (updated every 3 years)
- MMR: documented proof of two vaccines or positive titer; no declinations accepted
- Varicella: documented proof of immunity or two vaccines; no declinations accepted
- Tdap: valid within 10 years; no declinations accepted
- Hepatitis B: vaccination series completion or signed declination
- Influenza vaccine (optional; mask required if declined during flu season)
- Tuberculosis screening: 2-step PPD or IGRA within required timeframes, chest X-ray if positive, annual TB questionnaire
- Fit Test for respiratory protection (preferred before start, can be done onsite)
- Health statement or physical exam prior to first assignment, updated annually
- Drug Screening: 12-panel drug screen within 30 days prior to start, updated annually
- Skills and Competency:
- Ascension Core Competencies (HIPAA, Joint Commission, OSHA, etc.) with minimum passing score
- Ascension Skills Checklist for clinical specialty, updated annually
- Identification: Valid driver’s license or state ID matching address on file
- Badge photo: forward-facing headshot with white or blue background, no alterations
Additional Notes
- Contract length is approximately 13 weeks
- This role requires strong interdisciplinary collaboration and patient advocacy skills
- Emphasis on documentation accuracy and compliance with clinical and regulatory standards
