Job Summary

Seeking a Registered Nurse specialized in Operating Room (OR) for a long-term assignment.

Required Qualifications

• Valid RN license in Idaho
• Minimum of 1 year of Operating Room (OR) experience, with OR experience within the last year
• BLS certification issued by the American Heart Association
• ACLS certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Onboarding and Compliance Requirements

• Current and valid license/certification verification dated within 30 days of start.
• Fit test for respiratory protection.
• Negative 10-panel drug screen completed within 30 days of start (no THC).
• Signed Code of Conduct and Confidentiality & Security Agreements.
• Specialty-specific competency testing completed within the last year.
• COVID-19 vaccination proof including booster or approved exemption/declination (valid through October 2026).
• Proof of MMR vaccination or immunity.
• Hepatitis B vaccination series initiated prior to start.
• Influenza vaccine is optional.
• TB screening: 2-step PPD or IGRA with chest x-ray if positive (per regional requirements).
• Proof of Varicella vaccination or immunity.
• Physical/health statement confirming fitness to work within the last year.
• Criminal background check (federal, nationwide, county) within last 7 years, dated within 30 days of start.
• Education verification of highest academic degree or diploma.
• EPLS and OIG exclusion checks dated within 30 days of start.
• Integrity and audit services form signed shortly before start.
• Joint Commission CORE exam within the last year.
• National Sex Offender Registry (NSO) check dated within 30 days of start.
• System access form completed and uploaded one week before start.
• Skills checklist provided and completed for clinical contractors.

Additional Notes

• All certifications requiring images must include front and back of the card.
• All documentation must be uploaded to the credentialing system prior to or at onboarding.
• Declinations for vaccines are not accepted in California or Illinois (not applicable here but noted).
