Physical Therapist - Home Health
Location: Cleveland, Ohio
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Job Description
- Provide physical therapy services in a home health setting.
- Experience with home care required.
- Familiarity with Epic electronic health record system preferred.
Required Qualifications
- Valid Physical Therapist license in Ohio.
- Minimum of 5 years of experience as a Physical Therapist.
- Physical Therapist experience within the last year.
- Basic Life Support (BLS) certification issued by the American Heart Association.
- Experience in a Home Care setting.
- Experience with Epic electronic health record system.
Onboarding and Compliance Requirements
- Submission packet must include:
- Resume (education and work history)
- Two professional references from the last 2 years
- Completed job application form
- Employment history verification for the last 5 years (third-party verified)
- Background checks and screenings:
- Criminal background check within 6 months, including Social Security trace, county searches for last 7 years, national sex offender registry, and national criminal sweep.
- Ohio fingerprinting and FBI background checks depending on residency duration in Ohio. Attestation allowed if fingerprints pending, but results must be uploaded promptly.
- OIG exclusion check within 6 months prior to start.
- SAM/EPLS/GSA/OFAC screening within 6 months prior to start.
- Drug screen (10-panel) within 60 days prior to start; instant drug screens not accepted. New screen required if break in assignment exceeds 60 days.
- Completion of safety and compliance training modules prior to clearance; renewal required annually.
- Skills checklist (clinical-specific) completed within 1 year prior to start (one-time requirement).
- Competency exam with minimum 80% score within 1 year prior to start (one-time requirement).
- Core competencies exam (HIPAA, OSHA, Joint Commission topics) with minimum 80% score within 1 year prior to start (one-time requirement).
- Badge photo (headshot, shoulders up, plain background) required prior to onboarding.
- I-9 attestation confirming eligibility to work in the U.S.

- COVID-19 vaccination proof or approved medical/religious exemption preferred but not required.
- Influenza vaccine proof required during flu season (October–March) or completion of masking and declination forms if declining.
- Positive PPD requires clear chest x-ray valid for 5 years.
- Education verification required through third-party; international degrees require equivalency documentation.
