Speech Pathologist - Inpatient Rehab
Location: Lakeport, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Provide speech pathology services in an inpatient rehabilitation setting.
- Work schedule: Monday to Friday, 8:00 AM to 4:30 PM (5 days a week, 8-hour shifts).
- No call and no weekend shifts.
Required Qualifications
- Graduate of an accredited Speech Pathology program with a Master’s degree (M.A. or M.S.).
- Licensed or eligible for licensure as a Speech Pathologist in California.
- Basic Life Support (BLS) certification (AHA or ARC).
- New graduates are eligible to apply.
Background and Compliance Requirements
- 7-year background check including Social Security trace with all aliases, county searches, sex offender search, education verification, and nationwide checks.
- FACIS Level 3 screening required within 2 weeks of start and monthly thereafter.
- EPLS/GSA/SAM, HHS/OIG, OFAC sanctions, and Medi-Cal Suspended and Ineligible Provider List checks required and updated annually.
- Education verification of highest degree achieved is required.
- Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law).
- Physical exam within 1 year of start, including clearance for work and any necessary accommodations.
- TB screening questionnaire within 90 days of start, renewed annually, with negative TB test or chest x-ray as applicable.
- Immunizations or proof of immunity required for:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (series or declination)
- Varicella
- Seasonal flu vaccination or signed declination required during flu season (Nov 1 - Apr 30).
- Fit mask test within 1 year of start, renewed annually (with accommodations for religious exemptions or failed tests).
- Respiratory safety clearance required prior to start and renewed annually.
Documentation and Verification
- Must provide full 7 years of work history with no gaps, including city and state of employment.
- Employment verification required for up to 7 years; gaps over 30 days must be explained.
- Two professional references within the last 3 years required.
- Primary source license verification within 30 days of start.
- Official transcripts or diplomas may be required if education verification is unsuccessful.
- Candidate must review and sign facility-specific documents including job description, standards of business conduct, vendor training, code of conduct, overpayment refund policy, and regulatory orientation prior to start.
- Completion of a conditional offer of employment attestation is required before starting.
Additional Notes
- Candidates requesting religious exemptions for vaccines or facial hair must complete specific exemption forms and obtain approval.
- Candidates with positive TB history must provide a negative chest x-ray within 1 year of start.
- Candidates must comply with all facility-specific policies and training requirements.
