Description
Physical Therapist - Home Health (Travel/Contract)
Location: Dallas, Georgia
Contract Length: 13 weeks
Job Overview
- Provide physical therapy services in patients' homes with periodic office meetings.
- Average caseload: 5-6 patients per day (5-10 patients assigned daily, equating to 6-8 points per day).
- Types of cases: Start of Care and Follow Ups.
- Schedule: Monday to Friday, 8:00 AM to 5:00 PM, with rotation for weekend on-call every 4th weekend.
- Must be willing to travel up to 75 miles from the home clinic, covering multiple counties including Paulding, Powder Springs, Rockmart, and Acworth.
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient is not billable).
- No guaranteed hours; if patient census is low, contractor may be floated to a nearby clinic to pick up hours.
- Local candidates accepted at the same rate.
Required Qualifications
- Valid Physical Therapist license in Georgia
- Minimum of 1 year of Physical Therapist experience within the last year
- Experience working with adult patients
- Experience in a Home Health setting
- BLS certification
Work Environment & Expectations
- Home visits primarily; some periodic office meetings.
- Use of EMR systems: HomeCare HomeBase (HCHB), OASIS, Outlook.
- Weekend on-call rotation every 4th weekend (details vary, discuss during interview).
- Schedule may vary by location; some positions may have a 4x10 shift schedule (Friday-Monday), but most are standard 5x8 (M-F).
Compliance & Documentation Requirements
- Must complete and maintain all required background checks, including but not limited to:
- 7-year standard background check with county, nationwide, sex offender, and motor vehicle record searches.
- EPLS/GSA/SAM and HHS/OIG checks within 14 days of start and monthly thereafter.
- Fingerprinting as required by state regulations.
- State-specific background and registry checks as applicable.
- Required health documents:
- Physical exam within 12 months confirming good health and no communicable diseases.
- Negative TB test or chest X-ray within 12 months (no pending results at start).
- Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- 10-panel drug screen per Aya standard.
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or declination.
- Must complete all facility-specific compliance documents and training prior to start, including but not limited to:
- Confidentiality and privacy policies, HIPAA, code of conduct, compliance training, home health core policies, and COVID-19 policy attestation.
- Two professional references required.
- Verification of work history for at least the last 7 years or two most recent employers.
Additional Notes
- Weekend requirements and on-call expectations vary by location and should be discussed during the interview.
- Candidates must be prepared to sign new supplier packets at the start of each assignment and annually if working more than one consecutive year.
- Must be willing to comply with all state-specific and facility-specific requirements and documentation.
