    Description

    Occupational Therapist

    Location: Elk Grove, California
    Employment Type: Clinical-Allied
    Contract Length: 13 weeks
    Shift: Monday to Friday, 8:00 AM – 4:30 PM
    Hours per Week: 40 hours
    Transportation: Reliable transportation required for clinic-facing roles


    Job Summary

    • Provide restorative and rehabilitative Occupational Therapy services to participants both at the center and in their homes.
    • Collaborate closely with physicians, rehabilitation staff, and interdisciplinary team members to maximize participant independence and safety.
    • Conduct Occupational Therapy assessments to determine care needs, preferences, and goals.
    • Deliver and document interventions to promote muscle re-education, perceptual-motor and cognitive function, balance, endurance, and enhance performance of activities of daily living (ADLs).
    • Supervise Therapy Assistants, including training and guidance for implementing Functional Maintenance Programs (FMP).
    • Provide training and instruction to staff, participants, and caregivers/families related to Occupational Therapy.
    • Manage equipment ordering, tracking, and maintenance.
    • Communicate changes in participant condition or functional status promptly to the interdisciplinary team.
    • Support smooth care transitions between settings such as hospitals, skilled nursing facilities, and home.
    • Participate in end-of-life care coordination and support.
    • Manage direct reports through training, supervision, and mentoring.
    • Complete all required documentation and mandated trainings timely and accurately.
    • Maintain confidentiality and adhere to company policies and quality improvement activities.
    • Exercise flexibility as business needs evolve.

    Required Qualifications

    • Valid Occupational Therapist license in California
    • National Board for Certification in Occupational Therapy
    • Minimum of 2 years of Occupational Therapy experience, including experience within the last year
    • Basic Life Support (BLS) certification
    • Experience working with geriatric patients
    • Experience in supervisory roles
