Description
Clinical Nurse Navigator I
Location
- Costa Mesa, California
- 100% onsite
Employment Type
- Local Contract (Clinical Nursing)
- Contract length: 13 weeks (approximately 3 months initially, with potential extension based on coverage needs)
- Work schedule: Monday to Friday, 8:00 AM – 5:00 PM (40 hours/week)
Job Summary
- Coordinate seamless care for patients with complex, integration-sensitive diagnoses across the care continuum.
- Advocate for patients, coordinating care from diagnosis or admission through discharge or end of life.
- Educate patients and families on healthcare processes and available resources.
- Link patients and families to appropriate services as needed.
- Enhance patient experience both clinically and in service delivery.
- Assist in quality improvement initiatives by gathering, tracking, and researching quality metrics.
- Monitor outcomes such as patient satisfaction, treatment compliance, and loyalty.
- Collaborate with nursing staff and may orient/train incoming Clinical Nurses and Patient Care Assistants.
- Perform other duties as assigned.
Required Qualifications
- Valid Registered Nurse (RN) license in the State of California
- Minimum of 3 years of Care Manager experience, including experience within the last year
- Bachelor of Science in Nursing (BSN)
- Nurse Navigator experience
- Oncology nursing experience
- Stroke education
- Bilingual skills
