Job Opportunity: Physical Therapist - Outpatient
Location
- Mount Vernon, Ohio

Schedule
- Monday to Friday
- Day shift, typically 7:30 AM to 4:00 PM (8 hours per day, 40 hours per week)
- Hours may range from 7:00 AM to 7:00 PM with flexible scheduling to cover shifts
Job Description
- Full caseload with a variety of diagnoses
- Outpatient therapy/rehabilitation setting
Required Qualifications
- Valid Ohio Physical Therapist license
- Government-issued photo ID (must be carried during hours of service)
- Proof of E-Verify or Attestation
- Background checks including:
- 7-year criminal history check within one year of start, updated annually
- Social Security trace, county/state/nationwide criminal checks, sex offender registry, FACIS Level 3
- EPLS/GSA/SAM and HHS/OIG checks updated monthly or with each contract renewal
- FBI and BCI fingerprinting within 12 months of start
- Education verification from an accredited program (Bachelor’s degree in Science in Allied Health Profession required)
- Current certifications as applicable (see Certifications section)
- Physical exam within 12 months of start, updated annually
- Immunizations and health requirements:
- Negative TB test or chest X-ray within 1 year of start, updated annually
- Proof of vaccinations or titers for Measles (Rubeola), Mumps, Rubella, Varicella, Hepatitis B
- Tetanus/Diphtheria/Tdap vaccine (no declinations accepted)
- Seasonal flu vaccination or approved medical/religious exemption (exemptions require facility forms)
- Drug screening: 9 or 10 panel drug screen within last 12 months (not required annually)
- Proof of 2 recent clinical references within one year of start
- Completed employment application including 2 years prior clinical practice in acute care or similar area
- Unit-specific competency exams required and must be updated annually (Medsurg competency acceptable for Rehab)
- Skills checklist updated annually
Certifications
- Basic Life Support (BLS) certification required (ARC, AHA, or US Department of Defense)
- Advanced Cardiac Life Support (ACLS) as required (ARC, AHA, or US Department of Defense)
- Pediatric Advanced Life Support (PALS) and Neonatal Resuscitation Program (NRP) as required based on unit
- Other certifications as applicable to specialty and unit
Additional Requirements
- Completion of facility-specific onboarding documents and policies prior to start
- HealthStream modules must be completed promptly upon receipt during orientation
- Photo for badge identification required
- Auto insurance required for Home Health travelers (if applicable)
- For candidates working in specialized units (e.g., Cath Lab), radiation badge may be required

- Must carry valid government-issued photo ID and licensure at all times during assignment
- Background checks and credential verifications must be current and compliant with facility standards
