Registered Nurse – Med Surg Tele Unit
Location: Crescent City, California
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Summary
Provide direct nursing care for patients of all ages on a Medical-Surgical Telemetry unit. Manage care of assigned patients and direct ancillary staff activities. Perform assessments, plan, implement, and evaluate patient care plans. Demonstrate leadership skills, including charge nurse responsibilities.

- Day shift
- 3 shifts per week
- 12 hours per shift (7:00 AM – 7:00 PM)
- Total 36 hours per week
Required Certifications & Licenses
- California Registered Nurse (RN) License
- BLS (Basic Life Support) – AHA or ARC only
- ACLS (Advanced Cardiac Life Support) – AHA or ARC only
- PALS (Pediatric Advanced Life Support) – AHA or ARC only
Required Qualifications
- Valid RN license in California
- Associate of Science in Nursing / Associate Degree in Nursing
- Minimum 1 year of Med Surg / Telemetry experience within the last year
- Experience in charge nurse roles
- BLS (Basic Life Support) issued by American Heart Association or American Red Cross
- PALS (Pediatric Advanced Life Support) issued by American Heart Association or American Red Cross
Experience
- Specialty: Registered Nurse (Med Surg Tele)
- Required years of experience: Not explicitly stated; assume minimum RN competency for Med Surg Tele unit (typically 1+ years recommended)

Background & Screening Requirements
- 7-year background check within 6 months of start, including SSN trace with all aliases, county searches, sex offender search, education verification, and FACIS Level 3 screening
- EPLS/GSA/SAM and HHS/OIG checks required and updated annually
- Drug screening: Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids per CA law)
- Employment verification covering 7 years with no gaps over 30 days; must include city and state of employment
- References: 2 professional references required (within 3 years)
Health & Immunization Requirements
- Physical exam within 1 year of start, cleared for work
- Fit mask test within 1 year, renewed annually (specific mask types required)
- TB screening: Negative QGold or TSPOT within 90 days or 2-step PPD testing; renewed annually
- Immunizations or titers required for:
- Tetanus/Diphtheria/Tdap (within 10 years)
- Measles (Rubeola)
- Mumps
- Rubella
- Hepatitis B (series or declination)
- Varicella
- Seasonal flu vaccination required during flu season (Nov 1 – Apr 30) or signed declination with mask-wearing requirement
- Color blindness exam if applicable (in-person exam only)
- Respiratory safety clearance required annually
Competency & Testing Requirements
- Medication/Pharmacology competency exam as applicable
- Dysrhythmia/EKG competency exam may be required for Telemetry units
- Unit-specific skills checklist required, renewed annually
- ICU & Med Surg Tele RNs at certain locations may be required to take an onsite EKG exam after start

- TB questionnaire signed by candidate and healthcare provider (MD, PA, NP/APN, DO, or RN) within 90 days of start
- Conditional offer of employment attestation required prior to start
- Religious exemption forms required for facial hair (PAPR) or vaccine exemptions, approved on a case-by-case basis

- Hard copy or documentation of professional license required if online verification does not include license number
- Official transcripts/diplomas may be required if education verification is unsuccessful
- Candidates with positive TB history require a negative chest x-ray within 1 year of start
- Failed fit tests due to facial hair require shaving and retesting; other failures require PAPR training and documentation
- Candidates must have full 7 years of work history documented in the application with no gaps
