Description
Registered Nurse (RN) - Skilled Nursing Facility (SNF)
Location: Montour Falls, New York
Employment Type: Long-Term Assignment (LTA)
Shift: Evening (3:00 PM – 11:30 PM)
Contract Length: 13 weeks
Job Summary
- Serve as Professional RN Team Lead responsible for time management, critical thinking, delegation, and prioritization.
- Provide direct patient care including assessment, monitoring, recording, and reporting patient conditions.
- Collaborate with interdisciplinary care teams and advocate for patients.
- Ability to step down to an LPN Med Cart role as needed and staffing permits.
- Required to work every other weekend and every other holiday.
- May float to other units as staffing permits.
Required Qualifications
- License: Current, active RN license valid in New York State.
- Experience: Minimum 2 years of RN experience in Skilled Nursing Facility or related setting (based on specialty and typical expectations for RN SNF roles).
- Certifications:
- BLS (American Heart Association or American Red Cross), valid throughout assignment.
- Skills: Must complete and maintain unit-specific skills checklist and pass annual core competency exams with a score of 80% or higher.
Health and Safety Requirements
- Immunizations and Testing:
- MMR: Positive IgG titer or 2 vaccine series required; if negative/equivocal, vaccination series must be completed within 30 days of start.
- Varicella: Positive IgG titer or 2 vaccine series required; similar rules as MMR.
- Hepatitis B: Positive antibody titer, 3 vaccine series, or specific declination form required.
- Tdap: Must be current within last 10 years or have declination form.
- TB Test: Must have one of the following within last 12 months: 2 skin PPDs (one within 3 months), or 1 Quantiferon/Spot test; past positives require chest x-ray within 12 months.
- Fit Test: Annual respirator fit test required prior to start; on-site fit testing available if needed.
- Drug Screen: 9+ panel drug screen excluding marijuana (not permitted to be tested in New York).
- Physical/Health Statement: Completed within last 12 months, including clearance with no restrictions, vital signs, and vision screening.
Documentation and Compliance
- Valid U.S. driver’s license or state ID (unexpired).
- Background check including multi-county criminal searches, national criminal check, SSN address trace, and FACIS 3 check.
- Employment history covering last 7 years with supervisor references (1 from last 12 months, 1 from last 3 years).
- Education verification of initial nursing degree (ASN or BSN).
- Signed job description and acknowledgment of code of conduct, sexual harassment policy, and other required organizational forms.
- Color headshot photo (professional, clear, no filters).
- Resume with detailed employment and education history.
- Centralus-specific forms and checklists as required for onboarding.
Additional Notes
- Prior travel experience is not explicitly required.
- Must be able to work flexible shifts including holidays and weekends.
- Floating to other units may be required depending on staffing needs.
