    Job Opportunity: Case Manager (Registered Nurse)

    Location: Oakland, California
    Employment Type: Travel/Contract
    Contract Length: 13 weeks


    Job Summary

    Seeking an experienced Registered Nurse Case Manager with 5+ years of inpatient acute case management experience. The role involves care coordination and care transitions planning throughout the acute care patient experience, working collaboratively with physicians, utilization managers, social workers, bedside nurses, and other interdisciplinary team members to ensure timely and appropriate patient care transitions and resource utilization.


    Work Schedule

    • 5 days per week, 8-hour shifts (5x8)
    • Day shift: 8:00 AM – 4:30 PM
    • Every other weekend required

    Key Responsibilities

    • Conduct initial and ongoing patient assessments, including medical, psychosocial, and financial information gathering.
    • Lead healthcare team in developing cost-effective care coordination plans.
    • Oversee case management assistants in post-acute care service planning.
    • Perform utilization management by reviewing medical records to ensure appropriate level of care and expedite transitions.
    • Collaborate with physicians and interdisciplinary teams to prevent unnecessary admissions/readmissions.
    • Ensure compliance with Medicare and regulatory requirements related to care transitions.
    • Identify barriers to care progression and work with the team to resolve them promptly.
    • Manage complex clinical and social situations, including high utilizers of the Emergency Department.
    • Communicate with financial counselors regarding uninsured or underinsured patients.
    • Participate actively in department operations and continuous improvement initiatives.
    • Maintain professional relationships with healthcare team members and community resources.

    Required Qualifications

    • Valid Registered Nurse (RN) license in California
    • Minimum 5 years of acute care case management experience, including experience within the last year
    • Experience in inpatient and acute care settings
    • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
    • Advanced Cardiac Life Support (ACLS) certification issued by the American Heart Association or American Red Cross
    • Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
