Registered Nurse (Medical/Surgical) - Cleveland, Ohio

Job Details

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 13 weeks
• Shift: 7:00 PM – 7:30 AM
• Primary Responsibility: Direct patient care

Required Qualifications

• License: Valid permanent RN license (temporary licenses not accepted)
• Experience: Minimum 1 year of experience in Medical/Surgical nursing (default to higher if conflicting info)
• Certifications:

  • BLS (American Heart Association only)
• Immunizations and Health Requirements:

  • MMR: Positive titers within last 15 years or approved booster protocol
  • Varicella: Positive titers within last 15 years or approved booster protocol
  • Hepatitis B: Proof of full series, partial series with declination, or positive titers within 15 years
  • Tdap vaccination within last 10 years (no declinations accepted)
  • Influenza vaccine required during flu season (Oct-Mar) or completion of specific masking and declination forms
  • 2-Step TB test or IGRA (Quantiferon/Tspot) within last 6 months; if positive, clear chest x-ray and current TB questionnaire required
  • Fit Test for approved respirators completed within 1 year prior to start
• Health Screening:

  • Physical exam within 1 year prior to start confirming ability to work without restrictions
• Background and Verification:

  • Primary source verification of RN license and Nursys report within 30 days prior to start, updated every 2 years
  • Ohio fingerprinting and criminal background checks compliant with state and federal regulations (requirements vary based on length of Ohio residency)
  • National criminal background check including sex offender registry within 6 months prior to start
  • Social Security trace and other professional verifications within required timeframes
  • Employment verification for last 5 years through third-party verification (no attestations)
  • Education verification through third-party (no attestations); international education requires equivalency documentation
• Compliance and Testing:

  • Drug screen (10-panel) within 60 days prior to start; no instant tests allowed
  • Competency exams with minimum 80% score within 1 year prior to start
  • Core competencies (HIPAA, OSHA, Joint Commission topics) completed within 1 year prior to start with minimum 80% score
  • Skills checklist specific to clinical area completed within 1 year prior to start (initial hire only)
  • Completion of required safety and compliance training modules prior to clearance; renewal annually
• Documentation:

  • Non-expired photo ID (Driver’s License or State ID)
  • Professional resume including education and work history
  • Two professional references from within last 2 years
  • Job application and employment history form as required
  • Badge photo (headshot, plain background, JPEG format)
