Speech-Language Pathologist

Job Overview

Location: Live Oak, California
Setting: Skilled nursing facility
Employment type: Travel/contract
Start date: ASAP
Assignment length: 13 weeks preferred
Weekly hours: 36 hours guaranteed
Shift: Day shift, 5x8 schedule
Schedule notes: May include occasional weekend coverage and floating
Cancellation notice: 14 days

Required Qualifications

Valid Speech Language Pathologist license in California
1 year of Speech Language Pathologist experience
Speech Language Pathologist experience within the last year
Experience in a Long Term Care setting
BLS issued by American Heart Association or American Red Cross

Compliance / Onboarding

Emergency contact form before start
Standard background check
EPLS/GSA/SAM screening
HHS/OIG screening
CA wage and rehab hour quiz
Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year of start
Measles, mumps, and rubella documentation based on birth year requirements
Hepatitis B documentation
Varicella documentation
Drug screen within 1 year of start
Flu vaccination or declination during flu season
2 professional references within 3 years of start
