Required Qualifications
- Valid Physical Therapist license in California
- Physical Therapist experience within the last year
- BLS
- Experience in a Home Health setting
Responsibilities
- Prioritize start-of-care visits
- Supervise Physical Therapy Assistants
- Submit start-of-care reports by the next day
- Complete clinical notes and other documentation within 48 hours of the visit
- Maintain timely communication with the supervisor, intake, and office
- Be responsive during normal business hours

Contra Costa County, California, with expansion as needed

- 3 full days of training, including 1 in-house day and 2 field days

Reimbursement and Support
- Mileage reimbursement: $0.65.5 per mile from first to last patient
- Parking and tolls reimbursed

- Recertification, resumption of care, OASIS discharge, evaluation: 1.5 points
- Revisit and non-OASIS discharge: 1 point
