Hospice Nurse

Overview

Contract role in Sunnyvale, California
Day shift: 8:30am–5pm
40 hours per week
Schedule can be flexible based on patient availability
One weekend per month

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
Hospice experience
Case Management (1 year of experience)
Case Management experience within the last year

Role Summary

Admits patients to hospice service
Assesses patient and family physical, psychosocial, environmental, safety, and developmental needs
Develops individualized plans of care
Documents observations and interventions related to the patient’s environment
Coordinates care with a multidisciplinary team
Participates in quality assessment and performance improvement activities
Conducts follow-up visits as needed

Service Area

San Francisco, Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities as needed
