Description

Hospice Nurse

Overview

Location: Sunnyvale, California
Employment type: Contract
Contract length: 13 weeks
Schedule: Days, 9:30am–6:00pm; schedule can be flexible based on patient availability
Weekend requirement: One weekend per month
Service area: Millbrae, Burlingame, San Mateo, Foster City, Belmont, Redwood City, and nearby cities as needed

Role Summary

Admit patients to hospice service
Assess physical, psychosocial, environmental, safety, and developmental needs
Develop individualized plans of care
Document observations and interventions related to the patient’s environment
Coordinate care with a multidisciplinary team
Participate in quality assessment and performance improvement activities
Conduct follow-up visits as needed

Required Qualifications

Valid RN license in California
Associate of Science in Nursing / Associate Degree in Nursing
Hospice experience within the last year
1 year of hospice experience
BLS certification
