Required Qualifications

• Valid Occupational Therapist license in California
• Occupational Therapist (2 years of experience)
• Occupational Therapist experience within the last year

• Employment application with a full 7-year work history, including city and state for each location and no gaps
• Work history verification for up to 7 years, with any gaps over 30 days explained
• Background check, including 7-year history and alias/trace-based searches
• Sex offender search within 90 days of start
• Education verification
• Official transcript or diploma if verification cannot be completed after documented attempts
• TB screening within 90 days of start and annual renewal
• Physical exam within 1 year of start
• Fit test within 1 year of start and annual renewal
• Negative 9-panel drug screen within 6 months of start
• Flu vaccination documentation or signed declination during flu season
• Tdap within 10 years of start
• Measles, mumps, rubella, varicella, and hepatitis B immunity or vaccination documentation
• Candidate must review and sign facility onboarding documents before start
