Job Overview
- Location: Monmouth, Illinois
- Setting: OSF Holy Family Medical Center
- Shift: Days
- Schedule: Monday–Friday, 8:00 AM–4:30 PM, with some flexibility on start and end times
- Contract Length: 13 weeks
Required Qualifications
- BLS certification
- Active Illinois physical therapy license, or confirmation that the application process has been initiated
Submission Requirements
- Last four digits of SSN
- Middle initial
- Requested time off
Additional Notes
- Local placements are accepted within a 50-mile radius
- Candidates within 50 miles of the facility are subject to a local rate adjustment
- Drug screen must be negative for all panels
- 12 hours of non-billable orientation are included
- Call-off policy allows 12 hours every 13 weeks without penalty or shift repayment
