Description

Home Health & Hospice RN

Location

  • Dickinson, North Dakota

Contract

  • 13 weeks
  • Days, 8 hours per day, 40 hours per week

Required Qualifications

  • BLS certification
  • Home Health & Hospice experience
  • Current driver’s license
  • Current auto insurance

Schedule

  • Monday through Friday, 8:00 a.m. to 5:00 p.m.
  • On-call rotation for evenings, weekends, and holidays

Patient Population

  • Adults
  • Geriatrics
  • Pediatrics

Work Setting

  • Home health and hospice care
  • Service area includes homes, nursing homes, and other patient locations within a 60-mile radius of Dickinson

Key Responsibilities

  • Complete initial and ongoing patient assessments
  • Develop and revise care plans
  • Administer medications and treatments as prescribed
  • Provide patient and family education
  • Coordinate discharge planning
  • Serve as case manager when assigned
  • Prepare clinical notes and communicate with the physician as needed
  • Arrange needed equipment and services
  • Supervise home health aide care

Additional Notes

  • No clinical involvement
  • Mileage is included in the bill rate
  • Any solid color scrubs are allowed
  • RTO must be submitted upfront; additional RTO will not be approved
  • Candidates may not have been full-time, part-time, or PRN with any CommonSpirit-affiliated facility, including CHI or Dignity, within the past year
