RN Case Management
Job Overview
- Travel RN case management role in Paramount, California
- Full-time schedule: Monday–Friday, 8:00 AM–4:30 PM
- 40 hours per week
- Contract length: 12 weeks
Required Qualifications
- Valid RN license in California
- Acute Care Case Management (2 years of experience)
- Acute Care Case Management experience within the last year
- BLS issued by American Heart Association
- Associate of Science in Nursing / Associate Degree in Nursing
Role Focus
- Care coordination and utilization management
- Comprehensive assessment, care planning, implementation, and evaluation
- Discharge planning and continuity of care
