RN Care Manager

Overview

• Role: RN Care Manager
• Location: Chapel Hill, North Carolina
• Schedule: Days, 40 hours per week
• Contract length: 13 weeks

Summary

Provides ongoing support through assessment, planning, implementation, and evaluation of individual patient needs, with a focus on case management, utilization review, discharge planning, continuity of care, and cost effectiveness.

Required Qualifications

• Valid RN license in North Carolina
• Associate of Science in Nursing / Associate Degree in Nursing
• 2 years of Case Management experience
• Case Management experience within the last year
• Experience in an Acute Care setting
• Experience with discharge planning
• Care Manager experience

Preferred Qualifications

• BSN or MSN from an accredited school of Nursing
• Recent experience as a Case Manager, Care Manager, or Discharge Planner within an acute care hospital environment

Key Responsibilities

• Assess patient needs and document a plan of care in Epic
• Prioritize cases, identify new admissions, and monitor observation patients throughout the day
• Participate in daily care management touchpoints, CAPP meetings, and weekly Complex Care Meetings
• Coordinate with Social Work, Utilization Management, and the multidisciplinary team on discharge barriers, psychosocial concerns, and transition planning
• Support discharge readiness by identifying expected discharge dates, communicating medical milestones, and verifying understanding of the discharge plan
• Educate patients, families, and the care team on community resources and post-acute referrals
• Assist with medication resource management for non-resourced patients as needed
• Refer administrative tasks such as faxing and form processing to the Care Management Assistant
