Home Care Registered Nurse

Overview

• Contract role in St. Albans, Vermont
• Home Care and Hospice coverage needed
• 13-week contract
• 37 hours per week
• Shift: Sunday - Thursday, 8am-4pm
• On-call rotation and holidays required

Required Qualifications

• Valid RN license in Vermont
• Associate of Science in Nursing / Associate Degree in Nursing
• BLS
• Home Health experience within the last year
• Home Health (2 years of experience)
• Hospice experience

Responsibilities

• Assess patient and caregiver needs and develop realistic plans of care
• Serve as case manager for assigned patients and manage caseloads and visit times
• Implement medical plans of care, nursing interventions, and procedures
• Maintain accurate and timely nursing documentation
• Evaluate patient responses and overall quality of care
• Perform patient and caregiver education
• Identify interdisciplinary needs and communicate them to other disciplines
• Assess needs for HCA services and provide ongoing supervision
• Transmit patient incident reports as needed
• Ensure infection control standards are maintained
• Ensure supervisory visits are not combined with routine visits when necessary
