RNCC RN Care Coordinator (Care Coordination/Case Management)

Location: Carmichael, California Contract Length: 13 weeks Shift: Day, Evening (1200-2030, every other weekend; source also states hours between 8-5pm) Hours: 8 hours per day, 40 hours per week Unit: Case Management/Care Coordination (works in the Emergency Department) Scope: All areas of the hospital. No outpatient or home care. Status: Sent to manager for interview

Required Qualifications

• 2 years of experience, with a minimum of 2 years of hospital experience
• Active BLS certification
• Active California RN license
• Must have discharge planning and utilization management experience
• Emergency Department care coordination experience
• Must have prior recent California RNCC experience, or candidates will not be considered
• 2 supervisor references: 1 within the past 12 months and 1 within the past 3 years worked (charge accepted)
• Not directly employed (full-time, part-time, or PRN) by any facility associated with the health system within the past year. PRN candidates employed through outside firms will be considered.

Preferred Qualifications

• Interqual experience (highly preferred). Candidates will be trained in IQ.

Submittal Requirements

• Copy of driver's license required at submittal
• Name must match the government-issued ID, with no middle initial or middle name

Responsibilities

• Carries 15-20 patients seen/consults per day
• Works with all payers, uninsured, and homeless patients
• Completes initial assessments and IQ reviews
• Required skills: collaboration, teamwork, effective communication, and the ability to reprioritize throughout the day

Schedule and Work Environment

• Weekends: every other weekend depending on staffing, with at least one full weekend per month
• Holidays: not required, but available if willing
• Hospitalist or intensivist in house: Yes
• Respiratory therapy 24/7: Yes
• Pharmacy 24/7: No
• Attire: Business casual
• Orientation: 5 days of care coordination new employee orientation; unit orientation timeframe varies case by case
