Start Date: ASAP
We are seeking a skilled Registered Nurse Case Manager for a travel assignment in Dallas, TX. This role focuses on utilization review, discharge planning, treatment coordination, and resource and financial management to ensure patients receive the most appropriate, safe, and cost-effective care.
Key Responsibilities:

Conduct utilization reviews and coordinate with interdisciplinary teams on treatment plans and discharge processes

Ensure timely, accurate documentation in patient medical records

Identify barriers to discharge and develop safe, individualized plans of care
-
Maintain compliance with all regulatory, documentation, and payor guidelines
-
Engage with patients and families to provide education and support around care planning and transitions
-
Collaborate with physicians and departments to prevent unnecessary procedures or delays
-
Monitor and manage fiscal responsibilities, including contract levels and billing processes
-
Serve as liaison between facility, patients, families, insurers, and external agencies
-
Maintain current knowledge of community resources and support services
-
Step into Director-level duties as needed in their absence
Requirements:

Current RN license in Texas (or compact state)

Strong case management experience in acute or LTAC settings
-
Familiarity with UR processes, discharge planning, and payor requirements
-
Excellent communication, documentation, and critical thinking skills
-
Ability to work collaboratively with interdisciplinary teams and external stakeholders
