We are seeking a skilled Registered Nurse Case Manager for a travel assignment in Dallas, TX. This role focuses on utilization review, discharge planning, treatment coordination, and resource and financial management to ensure patients receive the most appropriate, safe, and cost-effective care.

Key Responsibilities:

• Conduct utilization reviews and coordinate with interdisciplinary teams on treatment plans and discharge processes

• Ensure timely, accurate documentation in patient medical records

• Identify barriers to discharge and develop safe, individualized plans of care

• Maintain compliance with all regulatory, documentation, and payor guidelines

• Engage with patients and families to provide education and support around care planning and transitions

• Collaborate with physicians and departments to prevent unnecessary procedures or delays

• Monitor and manage fiscal responsibilities, including contract levels and billing processes

• Serve as liaison between facility, patients, families, insurers, and external agencies

• Maintain current knowledge of community resources and support services

• Step into Director-level duties as needed in their absence

Requirements:

• Current RN license in Texas (or compact state)

• Strong case management experience in acute or LTAC settings

• Familiarity with UR processes, discharge planning, and payor requirements

• Excellent communication, documentation, and critical thinking skills

• Ability to work collaboratively with interdisciplinary teams and external stakeholders
