Travel RN Case Manager – LTAC

HumanEdge Health is seeking an experienced Travel RN Case Manager for a 13-week assignment at a Long-Term Acute Care (LTAC) facility in Hummelstown, PA. The Case Manager plays a vital role in managing patient care plans, ensuring quality outcomes, and coordinating safe, efficient discharges.

Key Responsibilities:

• Conduct utilization reviews, discharge planning, and treatment coordination

• Develop and manage individualized clinical plans of care

• Collaborate with the interdisciplinary team and physicians to resolve barriers to care

• Educate and involve patients and families in discharge and treatment plans

• Complete documentation accurately and in a timely manner

• Identify and refer patients to appropriate community resources

• Perform medical necessity reviews for admissions and continued stays

• Communicate with insurance providers to ensure timely authorizations and approvals

• Maintain awareness of payor requirements and regulatory standards

• Review billing practices to support reimbursement and minimize financial risk

• Assist with data collection, quality metrics, and internal reporting

• Attend case conferences and collaborate on care coordination

• Step into Director of Case Management duties as needed

Requirements:

• Active RN license in Pennsylvania or compact state

• Minimum of 2 years of acute care or LTAC case management experience preferred

• BLS certification required

• Strong knowledge of discharge planning, resource management, and payor guidelines

• Ability to work independently while collaborating with the broader clinical team

• Excellent communication, organizational, and problem-solving skills
