Description

  • Coordinate discharge plans, assess patient needs, identify resources, and arrange for follow-up care, working closely with medical staff and patients/families. 
  • Support utilization review and social service referrals. 
Experience Requirements:
•    Two years of inpatient/acute care discharge planning experience
•    Hospital’s bed size experience should be 250-300+ - notated on the jobs
•    Must be able to cover a variety of adult units with high acuity – ER experience is a plus
•    Epic Charting is required
•    Certs required: CCM preferred but not required. BLS required
•    Prefer RN, BSN, *Would consider ADN

For reference/context, this is UMMC’s campus pictured below. Can your case manager handle all of that?
•    Candidates must be experienced in this type of environment if they are to succeed.
•    UMMC is the only level I trauma facility in Mississippi and sees the worst of the worst cases
•    Indigent and homeless populations that are hard to discharge
•    Manage patients from L&D to the most critical adult ICU and everything in between
•    Teaching facility: Will also need to manage dealing with students, interns, residents, and families (where there is one)
•    Large campus, that’s super busy and fast paced
•    Short staffed so need case managers who can hit the ground running and help show how case managing is done.
•    No actual nursing responsibilities or utilization review.
