Job Description

  • Promote patient and employee safety with a focus on safe patient handling, materials handling, infectious diseases, slips/trips and falls, and work environment safety.
  • Identify patient problems affecting their ability to participate in treatment plans.
  • Assist patients and families in utilizing hospital resources and participating in decision-making for care plans and safe discharge planning.
  • Collaborate with interdisciplinary teams, including physicians, to manage clinical plans of care and facilitate safe, timely discharges.
  • Refer patients and families to outside agencies for social issues unrelated to healthcare as required.
  • Maintain communication with supervisors regarding major patient condition changes and emergencies.
  • Complete accurate and timely documentation, data collection, and statistical reporting.
  • Participate in team meetings and contribute to facility-wide needs and operational improvements.
  • Ensure medical necessity reviews and utilization reviews are completed according to guidelines.
  • Address barriers to patient care and discharge planning.
  • Maintain payor/referral satisfaction by meeting insurance certification and documentation requirements.
  • Keep an up-to-date community resource system to assist patients and families.
  • Collaborate with clinical teams to avoid unnecessary appointments, tests, and procedures.
  • Manage fiscal responsibilities related to billing and reimbursement.
  • Perform outlier management and attend relevant meetings.
  • Document plans of care and progress notes in compliance with policies and regulations.
  • Perform other duties as assigned.

Required Qualifications

  • Current licensure as a Registered Nurse (RN) appropriate for the Case Manager role.
  • Associate’s Degree in a healthcare field required; Bachelor’s Degree preferred.
  • Minimum 2 years of experience in Case Management and Discharge Planning within a Rehabilitation Care setting.
  • Working knowledge of the insurance industry and government reimbursement processes.
