Case Manager

Contract Length: 13 weeks

Job Description

• Responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management.
• Completes medical record documentation in accordance with policy and state regulations.
• Collaborates with interdisciplinary teams, including physicians, to manage and direct clinical plans of care.
• Assists patients and families in understanding and participating in their treatment plans and discharge processes.
• Identifies and addresses barriers to patient care and discharge.
• Ensures compliance with payor requirements and timely insurance certifications.
• Maintains up-to-date community resource information to support patient and family access to services.
• Works cooperatively with the clinical team to identify unnecessary appointments, tests, and procedures.
• Participates in meetings and contributes to facility-wide improvements.
• Manages outlier cases to optimize patient outcomes.
• Adheres to safety policies and promotes a safe work environment.

Required Qualifications

• Valid Registered Nurse (RN) license in Wisconsin.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Minimum of 2 years of case management experience, including experience within the last year.
• Experience in rehabilitation settings, including acute rehabilitation.
• Proficient in discharge planning.
• Working knowledge of the insurance industry and government reimbursement processes.
