Case Manager
Contract Length: 13 weeks
Job Description
- Responsible for utilization reviews, resource management, discharge planning, treatment plan management, and financial management.
- Completes medical record documentation in accordance with policy and state regulations.
- Collaborates with interdisciplinary teams, including physicians, to manage and direct clinical plans of care.
- Assists patients and families in understanding and participating in their treatment plans and discharge processes.
- Identifies and addresses barriers to patient care and discharge.
- Ensures compliance with payor requirements and timely insurance certifications.
- Maintains up-to-date community resource information to support patient and family access to services.
- Works cooperatively with the clinical team to identify unnecessary appointments, tests, and procedures.
- Participates in meetings and contributes to facility-wide improvements.
- Manages outlier cases to optimize patient outcomes.
- Adheres to safety policies and promotes a safe work environment.
Required Qualifications
- Valid Registered Nurse (RN) license in Wisconsin.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum of 2 years of case management experience, including experience within the last year.
- Experience in rehabilitation settings, including acute rehabilitation.
- Proficient in discharge planning.
- Working knowledge of the insurance industry and government reimbursement processes.
