Case Manager
Job Overview
The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, financial management, and completing medical record documentation. This role involves collaboration with interdisciplinary teams to ensure safe and timely patient discharges and effective care coordination.
Key Responsibilities
- Promote patient and employee safety by adhering to safety policies and procedures.
- Identify patient problems affecting treatment participation and assist in overcoming barriers.
- Assist patients and families in decision-making and development of safe discharge plans.
- Collaborate with physicians and clinical teams to manage clinical plans of care.
- Refer patients and families to appropriate outside agencies as needed.
- Conduct and document team and family conferences professionally.
- Ensure compliance with medical necessity review criteria and utilization review standards.
- Maintain up-to-date community resource information to support patient access to services.
- Work with clinical teams to identify unnecessary appointments, tests, and procedures.
- Manage payor requirements and insurance certifications to promote satisfaction and repeat business.
- Maintain fiscal responsibilities related to billing and reimbursement.
- Document plans of care and progress notes according to policy and regulations.
- Participate in facility-wide needs assessments and operational improvements.
- Attend scheduled meetings and complete assigned reports and duties.

Contract Length
- 13 weeks
Required Qualifications
- Current licensure as a Registered Nurse (RN) or other clinical discipline license as per state guidelines.
- Associate’s Degree in a healthcare field required; Bachelor’s Degree preferred.
- Minimum 2 years of experience in Case Management and Discharge Planning within a rehabilitation care setting.
- Working knowledge of the insurance industry and government reimbursement processes.
