Minimum Requirements:

• Registered Nurse (RN) License for NY or Compact as applicable
• Applicable recent experience in this RN Case Management work setting
• Commitment to quality care and adherence to client legal and client protocols
• Certifications: BLS, NY License
• Equipment/Procedures: EPIC, Parachute, Careport, Case Management, Discharge plan, Navigation Plan, Education patients post-surgery
• Weekend/Call: Weekend = None stated; Call = None stated
• Experience: 3 years
