Description

RN Case Manager

Location: Racine, Wisconsin
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks
Shift: 8:00 AM – 4:30 PM


Job Description

  • Provide RN case management services.
  • Care management experience required.
  • May need to float onsite within skill set or to other sites within a 40-mile radius as needed.

Required Qualifications

  • Valid RN license in Wisconsin.
  • Minimum of 1 year of case management experience within the last year.
  • Basic Life Support (BLS) certification issued by the American Heart Association (AHA).
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Valid Driver’s License.

Onboarding and Compliance Requirements

  • Completion of specialty competency exam with a minimum passing score of 80%.
  • Completion of core competencies including HIPAA, Joint Commission, NPSG, OSHA topics with minimum 80% passing score.
  • Color vision test completed prior to start and updated every 3 years.
  • Health statement or physical exam completed prior to first assignment and updated annually.
  • Fit test for respiratory protection (3M 1860 Regular, 1860 Small, or 1870+) preferred before start; can be completed onsite.
  • Documentation of immunizations and health screenings:
    • Two MMR vaccines or positive IGG titer (no declinations accepted).
    • Hepatitis B vaccination series completion or signed declination.
    • Two Varicella vaccines, serological evidence of immunity, or documented history (no declinations accepted).
    • Tdap vaccine valid within 10 years (no declinations accepted).
    • Influenza vaccine is optional; if declined during flu season, mask wearing is required.
    • Tuberculosis screening: 2-step PPD or IGRA within required timeframes, with chest X-ray if positive.
  • 12-panel drug screen within 30 days prior to start and annually thereafter.
  • Criminal background check including Social Security Trace, OIG Search, National Sex Offender Registry, and others within 90 days prior to start.
  • Employment and education verifications completed within 90 days prior to start.
  • Completion of onboarding education attestations and training modules including Epic test-out and specialty-specific skills checklist.

Documentation Required for Submission

  • Professional resume/profile.
  • Two professional references.
  • Nursys license verification.
  • Driver’s license or state ID.

  • Must be able to work onsite and travel up to 40 miles to other sites if needed.
  • All certifications and verifications must be current and compliant with specified timelines.
  • Some onboarding requirements must be completed prior to or at the start of the assignment.
