Registered Nurse (RN) - Home Health
Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 8:00 AM – 4:30 PM
Job Summary
As a Home Health RN, you will:
- Assess patient conditions and implement care plans.
- Provide ongoing education and support to patients and their families.
- Perform admission processes and case management duties.
- Deliver patient care aligned with community health practice standards.
- Utilize thorough and timely electronic documentation and interdisciplinary communication.
- Travel to assigned cases as needed.
- Participate in weekend rotations approximately once every 6 weeks.
Required Qualifications
- Valid RN license in Massachusetts.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Minimum 1 year of Home Health experience within the last year.
- Experience in a Home Health setting.
- Minimum 1 year of Case Management experience.
Required Documentation and Compliance
- Professional license verification with primary source validation (NURSYS and Board of Nursing required).
- Submission of a recent color photo (taken within past 6 months, plain background, no filters or eyeglasses).
- Valid TB test documentation within 3 months of start date (skin test or blood test), updated if there is a break >6 months.
- Fit test for N95 respirator within 365 days of start date, updated annually (no exemptions allowed).
- Respirator Medical Clearance attestation completed prior to fit testing.
- Documentation of MMR vaccination or immunity proof.
- Physician health statement signed by MD/DO/NP/PA within 365 days of start date, updated if >6 month break.
- Criminal background check within 45 days prior to start, including Massachusetts CORI and national sex offender registry checks, updated if >6 month break.
- Motor Vehicle Record (MVR) check within 45 days prior to start.
- Nurse Aide Registry check within 30 days of start (required for all employees).
- Education verification of highest level achieved.
- Social Security Number trace as part of background check.
- Exclusion checks against OIG, GSA, MassHealth, and NH Medicaid lists within 30 days prior to start and annually.
- Two professional references within the last 4 years, including at least one supervisor or higher.
- Skills checklist completed within 1 year of submission.
- Resume including past 7 years of work experience with city and state details.
- Candidate submission coversheet for initial assignment.
- Confirmation of assignment form upon acceptance and for any extensions.
Additional Information
- Hepatitis B vaccination or immunity is strongly preferred but optional.
- Tdap vaccination within 10 years is preferred but optional.
- If positive TB test, documentation of a negative chest x-ray after the positive test is required.
- Name change documentation required if legal name differs from credential or profile.
