Description

CT Technologist - Westerville, Ohio

Job Details

  • Employment Type: Long-Term Assignment (LTA)
  • Contract Length: 13 weeks
  • Shift: Weekends (Friday, Saturday, Sunday), 12-hour shifts from 5:30 PM to 6:00 AM

Required Qualifications

  • Valid license/certification as a CT Technologist
  • American Heart Association BLS certification (attach front and back of card)
  • Specialty exam completed within the past year
  • Proof of COVID-19 vaccination or approved exemption/declination (booster required by September 2025)
  • Proof of MMR vaccination or immunity
  • Proof of Hepatitis B vaccination series (titers not accepted)
  • Proof of Varicella vaccination or immunity
  • Influenza vaccine is optional but recommended (declination accepted)
  • 2 Step PPD or IGRA (Quantiferon/Tspot) test required (within last 5 years or 1 year if positive TB)
  • Fit test required
  • Physical/Health statement from exam within the last year confirming fitness to work
  • 10-panel drug screen completed within 30 days of start (NO THC)
  • License/certificate verification dated within 30 days of start
  • Color vision test required

Professional and Compliance Requirements

  • Resume and skills checklist within the past year
  • Two professional references
  • Confidentiality and information system security agreement signed within 30 days prior to start
  • Conflict of interest disclosure form signed within 30 days prior to start
  • Criminal background check (federal, nationwide, county for last 7 years) dated within 30 days of start
  • Education verification (highest level of academic education)
  • EPLS (Excluded Parties List System) check dated within 30 days
  • GSA/OIG exclusion check dated within 30 days
  • Integrity and audit services form signed within 30 days prior to start
  • Joint Commission CORE exam within the past year
  • National Sex Offender Registry check dated within 30 days
  • System access form completed and uploaded one week before start
  • Code of conduct form signed within 30 days prior to start
  • I9 form completed by agency

Additional Information

  • All required documentation must be uploaded to the credentialing system prior to or at onboarding as specified
  • Declinations for vaccines are not accepted for California or Illinois (not applicable here but noted)
