Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Location: Duluth, Georgia

Job Description

  • Provide physical therapy services in a home health setting, including home visits and periodic office meetings.
  • Average patient load: 5-6 patients per day.
  • Types of cases: Start of Care and Follow-Ups.
  • Schedule: Monday to Friday, 8:00 AM to 5:00 PM (some locations may have alternate schedules such as 4x10 shifts Friday-Monday).
  • Travel: Must be willing to travel up to 75 miles from the home clinic; typical travel is 30-40 miles per day, 50+ miles weekly.
  • Productivity expectations: 25-30 weekly units.
  • EMR systems used: HomeCare HomeBase, Oasis, Outlook.
  • On-call requirement: Yes, approximately 1 weekend per month.
  • Local candidates accepted at the same rate.
  • No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged.
  • Potential for hire at the end of the contract depending on fit and availability.

Requirements

  • Minimum 1 full year of recent adult home health experience required.
  • First-time home health therapists not accepted.
  • Must have a current, valid license in hand (pending licenses accepted only if they can be obtained within a few days).
  • Certifications required: CPR and BLS (Basic Life Support).
  • Reliable transportation and current driver's license required.
  • Agency-required liability insurance required.
  • Willingness to discuss weekend and on-call requirements during interview.

Compliance and Documentation

Candidates must complete and maintain the following (as applicable):

  • Valid Driver’s License/State ID.
  • Background checks including:
    • 7-year history verification.
    • Sex offender search.
    • EPLS/GSA/SAM and HHS/OIG checks.
    • State-specific background checks as required.
  • Fingerprinting (state-specific requirements apply).
  • Education verification.
  • Drug testing (10-panel screen).
  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months.
  • Immunization or titer proof for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B
    • Varicella
  • Seasonal vaccinations or declinations for Flu and COVID-19.
  • Completion of facility-specific compliance documents and training, including but not limited to:
    • HIPAA and Privacy Policy
    • Code of Conduct
    • Compliance Training Certification
    • Confidentiality Forms
    • Home Health Scenarios and Core Policy Acknowledgements
    • COVID-19 Policy and Attestation
  • References: Two professional references required.
  • Work history verification for the last 7 years or at least two most recent employers.

Additional Notes

  • Mileage reimbursement follows IRS standard rates; compensation is from patient home to patient home, not including commute to/from first and last patient.
  • Candidates must be prepared to sign new supplier packets at the start of each assignment and annually if working more than one year consecutively.
  • Some state-specific forms and attestations may be required depending on assignment location.
  • Candidates should be prepared to review and sign various compliance and policy documents prior to start.
