Licensing & Certification Requirements

• New York RN License: Must have license in hand before start
• Certifications (as required by unit):

  • ACLS (AHA Provider/Instructor/RQI acceptable)
  • BLS (AHA Provider/Instructor/RQI acceptable)
  • PALS (AHA Provider/Instructor/RQI acceptable)
  • NRP (Neonatal Resuscitation Procedure)
  • TNCC (Trauma Nursing Core Course)
  • EFM (Electronic Fetal Monitoring)
  • NIH Stroke Certification (annual update required)
  • State License Verification (Primary Source Verification within 30 days of start)
  • NURSYS Verification (within 30 days of start for RN)
  • Tech Cert Verification (if applicable, within 30 days of start)
  • Orientation Safety Manual Acknowledgement (required)

Background & Verification Requirements

• 7-year criminal background check (felony checks) within 3 months of start
• EPLS/GSA/SAM and HHS/OIG checks within 3 months of start, updated monthly
• Education verification from primary source (hard copies not accepted)
• Foreign equivalency education evaluation if applicable
• Transcript/Diploma if no education verification on file
• Work history verification (at least 2 past employments for direct candidates)
• References (2 within 3 years) required for direct candidates only
• Employment application and annual skills checklist required

Health & Compliance Requirements (Must be completed within 5 days of contract signing)

• Physical exam (within 1 year, updated annually)
• Fit mask test (annual; acceptable masks: Halyard duckbill 1870+, 1860 Regular, 1860 Small)
• Negative TB test or chest X-ray (within 1 year) or TB questionnaire annually
• Immunizations or titers for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (surface antibody positive titer, completed series, or declination)
  • Varicella
• Drug screening (within 30 days of start)
• Seasonal flu vaccination or signed declination (deadline December 3)
• Eye exam (Snellen, within 1 year)
• COVID-19 vaccination documentation preferred but not required

Facility Specific Requirements

• Government issued photo ID (Driver’s License or State ID)
• Passport-style photo for badge
• Completion of facility-specific online modules (approx. 4-6 hours) prior to start
• Completion of pre-employment compliance and health records attestations
• New York State specific checks as applicable:

  • NYS Nurse Aide Registry (if applicable)
  • NYS Criminal Background Check Fingerprint Results (for certain units)
  • NYS Disciplinary Action Search (for certain units)
  • NYS Sex Offender Registry Search (within 30 days of start)
  • NYS Medicaid Exclusion List check (within 3 months of start, updated monthly)

• Candidates must be located more than 75 miles from the facility to qualify
• First-time travelers are accepted for this assignment
