Registered Nurse (Travel/Contract) - Home Health
Location: Fredericksburg, Virginia

Job Description
- Provide, coordinate, and direct home nursing care according to physician orders and agency policies.
- Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits with periodic office meetings.
- Average patients per day: 5-8 (assigned 5-10 patients/day equating to roughly 6-8 points).
- Types of cases: Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, current driver's license, and agency-required liability insurance.
- Mileage reimbursed at IRS standard rate; compensated from patient home to patient home (commute to/from first and last patient not billable).
- Willingness to travel up to 75 miles from home clinic; average travel 40-50 miles/day, up to 100 miles/day possible.
- Schedule: Monday-Friday, 8 AM - 5 PM.
- Weekend rotation: 1 weekend per month.
- On-call: 1 weekend per month (varies by location).
- Dress code: Scrubs.
- Charting: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (used within last 6 months or retraining required).
Requirements
- Minimum 1 full year of recent Adult Home Health or Hospice experience (depending on specialty).
- First-time travelers not accepted; must have recent specialty experience.
- CPR and BLS certification required (AHA, National CPR Foundation, or ARC accepted).
- Must have an active nursing license (no pending licenses accepted).
- Proficiency in HCHB and OASIS documentation systems.
- Must be willing to cover a territory within a 50-mile radius of Fredericksburg, VA.
Productivity Expectations
- Weekly productivity units: 30-32 points.
Background and Compliance Requirements
- Valid driver's license and personal/auto insurance required.
- Comprehensive background checks including:
- 7-year history verification (work, residence, education).
- State-specific criminal background checks and sex offender registry searches.
- Federal and state exclusion lists (EPLS/GSA/SAM, HHS/OIG).
- Fingerprinting as required by state regulations.
- Health documents required:
- Physical exam within 12 months prior to start.
- Negative TB test or chest X-ray within 12 months.
- Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Drug screening (10-panel).
- Seasonal flu vaccination or documented declination.
- COVID-19 vaccination series or declination.
- Education verification and work history verification required.
- Two professional references required.
Additional Notes
- Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8, Monday-Friday.
- Local candidates accepted at the same rate.
- Weekend and on-call requirements vary by location and should be discussed during interview.
- Candidates must complete various compliance, confidentiality, and policy acknowledgments prior to start.
- Candidates must complete required training and quizzes related to compliance, home health scenarios, hospice policies (if applicable), and COVID-19 policies.
This position is ideal for experienced Registered Nurses with recent home health or hospice experience who are comfortable with travel within the specified radius and proficient in required documentation systems.
