Position Details

  • Employment Type: Travel/Contract
  • Contract Length: 13 weeks
  • Start Date: Approximately 3-4 weeks from compliance clearance
  • Shift Options: 36 or 48 hours per week
  • Patient Ratios: 1:3-5
  • Weekend Requirement: Every other weekend

  • Local Radius Rule: Home address must be at least 75 miles from facility

    • Acute Coronary Syndrome (ACS-STEMI/NSTEMI)
    • Heart Valve Disease
    • Coronary Artery Disease (CAD)
    • Pulmonary Hypertension
    • Hypertensive Crisis
    • Chest pain, shortness of breath, syncope
    • Pacemaker/AICD malfunction
    • Acute renal failure
    • Care of pulmonary hypertension patients

  • EPLS/GSA/SAM and HHS/OIG quarterly checks
  • Education verification of highest nursing degree
  • State-specific searches and registries as required
  • Fingerprinting required for Illinois long-term care or rehab units (not for IL home care)

Certifications

  • ACLS and BLS (AHA only)
  • Other certifications may be required depending on unit (e.g., PALS, NRP, TNCC, CPI/MAB, NIH Stroke Certification) – check unit-specific requirements
  • IV Certification required for LPNs/LVNs in Missouri
  • Medication/Pharmacology competency exams and unit-specific competency exams required and must be updated annually
  • Dysrhythmia/EKG competency exam required for RNs in specialty areas, updated annually

Health Documents

  • Physical exam within 1 year of start
  • Fit mask test (N95) required and must be renewed annually
  • Tdap vaccine or declination (expires every 10 years)
  • Negative TB test or chest x-ray per facility policy
  • Required immunizations or proof of immunity: Rubeola (Measles), Mumps, Rubella, Varicella
  • Drug testing within 1 year of start, must include specific panels and be current throughout assignment
  • Seasonal flu vaccination or approved exemption required during flu season (09/30 - 04/01)

  • HIPAA Acknowledgement, Latex Allergy Questionnaire, Confidentiality Form, Regulatory Compliance Education, Hep B Declination, TB Questionnaire, Hand Hygiene Policy, Fingernail Policy, Dress Code Policy, Orientation Modules (approx. 16 hours via WorkRamp)
  • Badge photo required (head and shoulders, white background, no obstructions)
  • Vehicle registration form required for day shift workers

References & History
