Job Opportunity: Registered Nurse - Home Hospice Case Manager

Location: Williamsburg, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Role: Home hospice case manager
• Must have experience with home hospice case management, no exceptions
• Patient census: approximately 35-45 patients
• Current BLS certification required

Required Experience

• Specialty: Registered Nurse (RN)
• Minimum years of experience: Not explicitly stated beyond hospice case management experience; assume at least 1+ year in home hospice case management based on job specificity
• Prior travel experience: Not required (no mention of prior travel experience requirement)

Certifications & Licenses

• BLS (Basic Life Support): Required (AHA, ARC, Military & American Safety & Health Institute; online CPR NOT acceptable)
• Other certifications (ACLS, PALS, NRP, EFM, NIH Stroke Certification) may be required depending on unit but not explicitly required for this role
• Valid and current driver’s license required (for all Home Health & Hospice positions)
• State RN license valid in Virginia

Background & Compliance Requirements

• 7-year criminal background check within one year of start, updated annually
• DMV driving record with minimum 3 years driving experience required
• Virginia State Police Check required prior to start
• Drug screen within last 12 months required
• Physical exam within last 12 months required
• Negative TB test or chest X-ray within last 12 months required
• Immunizations/vaccinations required:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (series, titer, or declination)
  • Varicella
  • Seasonal Flu vaccination during flu season (10/1-3/31) with strict deadlines and exemption policies
  • COVID vaccination preferred

Documentation & Other Requirements

• I9 verification required
• Government-issued photo ID required
• Photo for badge (recent, professional, specific guidelines) required
• Confidentiality agreement, dress code policy, scrub colors, safety brochure, drug-free policy, hand washing hygiene policy, and sworn disclosure must be reviewed and signed prior to start
• References: 2 professional references required
• Employment application/resume required
• Skills checklist must be updated annually

Additional Notes

• Must be at least 21 years old and 25 if transporting patients
• Must complete competency exams and medication/pharmacology tests as applicable within one year of start date
• Orientation includes pass/fail computer-based learning (CBL) tests
• Facility-specific policies and documentation must be completed prior to start
