Job Opportunity: Physical Therapist - Home Health (Travel/Contract)

Job Description

• Provide physical therapy services in patients' homes and periodic office meetings.
• Average caseload: 5-6 patients per day (5-10 patients assigned daily, equating to roughly 6-8 points per day).
• Types of cases: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8 AM to 5 PM (some positions may have alternate schedules such as 4x10 shifts Friday-Monday).
• Must be willing to travel up to 75 miles from home clinic; typical weekly travel up to 200 miles.
• Visits may include both patient homes and care facilities where Home Health can be safely provided.
• On-call required at least once per month.
• Local candidates accepted.

Required Qualifications

• Valid Physical Therapist license in Georgia.
• Minimum 1 year of recent experience as a Physical Therapist, specifically within the last year.
• Experience working with adult patients in a Home Health setting.
• Current BLS certification.

Productivity Expectations

• Weekly productivity expectation: 30 units.
• EMR systems used: HomeCare HomeBase, Oasis, Outlook.

Additional Requirements

• Must complete standard background checks including:

  • 7-year work/living history verification
  • Sex offender and state-specific registry checks
  • Drug screening (10-panel)
  • Fingerprinting as required by state regulations
• Health documents required prior to start:

  • Physical exam within 12 months confirming good health and no communicable diseases
  • Negative TB test or chest X-ray within 12 months
  • Proof of immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella
  • Flu vaccination or documented declination for current season
  • COVID vaccination series or documented declination
• Must review and sign various compliance, confidentiality, and policy documents prior to start.

Notes

• This is a contract assignment with the option for hire at the end if it is a good fit.
• Weekend requirements and on-call specifics vary by location and should be discussed during the interview.
• Mileage reimbursement follows IRS standard rates; compensation is from patient home to patient home, not commute to/from first and last patient.
