Job Description

Home health case management experience required
Proficiency with OASIS (Outcome and Assessment Information Set) required
Minimum 1 year of nursing experience required

Required Qualifications

Valid Registered Nurse (RN) license in Virginia
Minimum 1 year of case management experience within the last year
Experience in a home health setting
Proficiency and experience with OASIS (Outcome and Assessment Information Set)
Basic Life Support (BLS) certification issued by the American Heart Association or American Red Cross
Neonatal Resuscitation Program (NRP) certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Orientation Safety Manual acknowledgement
Acknowledgement of Pass/Fail testing requirements
