Case Manager (Registered Nurse) - Travel/Contract

Location: Portland, Oregon

Shift: Day (5x8 hours), 8:00 AM - 4:30 PM

Job Overview

• Position floats between units as needed.
• Weekend work required every other weekend.
• No on-call or block schedules required.
• Patient ratio approximately 20:1.
• Must have recent acute care experience in hospital care management/discharge planning (behavioral experience does not count).
• Variable schedule with flexibility to support vacation and PTO utilization.
• Vehicle required for assignment due to floating between multiple nearby facilities.
• Facility type: Short Term Acute Care hospital.
• Charting system: Epic.
• Parking: No fee.

Required Qualifications

• Valid Registered Nurse (RN) license in Oregon.
• Minimum 3 years of case management experience, including acute hospital case management and discharge planning.
• Case management experience within the last year.
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
• Experience in an inpatient setting.
• Care manager experience.

Additional Information

• Floating may include multiple facilities within the region; hotel accommodations provided if floating consecutive shifts to certain locations.
• No 48-hour shifts approved.
• Must be comfortable with variable schedules and floating to meet staffing needs.
• Facility provides scrubs or allows any color scrubs depending on unit.
• Badge photo required for identification.

Summary

This is a 26-week travel contract for a Case Manager RN with at least 3 years of inpatient acute care management experience. The role requires flexibility to float between units and facilities, weekend work every other weekend, and adherence to comprehensive credentialing, health, and training requirements. The position is based in a short-term acute care hospital environment using Epic for charting.
