Job Opportunity: Home Health Registered Nurse

Location: Hicksville, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Average patient load: 5-8 patients per day.
  • Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from the home clinic, averaging 250-300 miles per week.
  • Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours). Weekend and on-call duties as needed.
  • Use of HomeCare HomeBase (HCHB) EMR required; proficiency in OASIS required for RNs.

Required Qualifications

  • Minimum 1 full year of recent Adult Home Health or Hospice nursing experience (whichever specialty applies).
  • CPR and BLS certification current and valid.
  • Valid Registered Nurse license for New York State (license must be in hand; pending licenses not accepted).
  • Reliable transportation, current driver’s license, and agency-required liability insurance.
  • Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last use was over 6 months ago.
  • Must be able to meet documentation and travel requirements.
  • First-time travelers not accepted; recent specialty experience required.

Work Environment & Expectations

  • Dress code: Scrubs.
  • Patient visits primarily in home health settings within Nassau County territory.
  • No guaranteed hours; workload may fluctuate. Options to float to nearby clinics if hours are low.
  • Weekend and on-call requirements vary; discuss during interview.

Compliance & Documentation Requirements

  • Background checks including 7-year history, sex offender, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required prior to start (specific to New York and other states as applicable).
  • Education verification and work history verification (minimum 2 most recent employers or last 7 years).
  • Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, and seasonal flu vaccination or declination.
  • Drug screening: 10-panel drug test required.
  • COVID-19 vaccination series or declination required.
  • Various state-specific and facility-specific compliance documents and attestations must be reviewed, signed, and completed prior to start.

Additional Notes

  • Local candidates accepted at the same rate.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
  • Some positions may have alternate shift schedules (e.g., 4x10s Friday-Monday), but most are standard 5x8 Monday-Friday.
  • Candidates must complete annual compliance and training requirements if working more than one year consecutively.

This position is ideal for experienced Home Health RNs with recent specialty experience who are comfortable with travel within the assigned territory and meeting comprehensive compliance standards.
