Outpatient Physical Therapist - Contract Position

Location: Buckeye, Arizona
Employment Type: Contract
Contract Length: 13 weeks

Job Details

• Work Schedule: Days, 5 days per week, 8 hours per day (40 hours/week)
• Flexible full-time schedule possible (4x10 hour shifts)
• Patient Age Group: Adults and Adolescents
• Caseload: 25-30 cases per week
• Visits per Day: 10-15
• Dress Code: Business Casual (Khakis and polos)
• Electronic Health Record (EHR) System: NextGen

• Specialty: Rehabilitation Therapy / Physical Therapy
• Professional Skills: Case Management, Screening, Building Caseload
• Evaluation/Assessment Methods: Fall Risk Assessment, Gross Motor Assessments

Required Qualifications

• Valid Physical Therapist license in Arizona
• Minimum 2 years of experience as a Physical Therapist, including experience within the last year
• BLS certification
• Experience in case management
• Experience in screening and building caseload

Additional Information

• Productivity expectations based on visits per day and caseload size
• No prior travel experience required
