Job Opportunity: Home Health Registered Nurse

Location: Chesapeake, Virginia
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical/Vocational Nurses.
  • Average patient load: 5-8 patients per day.
  • Types of visits include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from the home clinic; average weekly travel is approximately 200 miles.
  • Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
  • Weekend rotation: approximately 1 weekend per month.
  • On-call requirement: approximately 1 week per month.
  • Charting system: Proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).
  • Dress code: Scrubs.

Required Qualifications

  • Minimum 1 full year of recent Adult Home Health or Hospice nursing experience (depending on specialty).
  • CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
  • Valid nursing license in hand (no pending licenses accepted).
  • Reliable transportation, current driver’s license, and agency-required liability insurance.
  • Proficiency with HomeCare HomeBase (HCHB) within the last 6 months; retraining required if last used over 6 months ago.
  • Must have completed at least one prior travel contract is not required (first-timers not accepted; recent specialty experience mandatory).

Compliance and Documentation Requirements

  • Background checks including 7-year history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required per state regulations (varies by state).
  • Education verification for nursing degree.
  • Physical exam within 12 months prior to start, confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Immunization or titer proof for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 vaccination or declination.
  • 10-panel drug screen required.
  • Seasonal flu vaccination or documented declination required.
  • Various state-specific forms and attestations depending on assignment location.
  • Candidates must review and sign multiple compliance documents prior to start, including confidentiality, HIPAA, privacy policies, and home health core policies.

Additional Information

  • Local candidates accepted at the same rate.
  • Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
  • Some positions may follow a 4x10 schedule (Friday-Monday), but most are standard 5x8.
  • References: Two professional references required.
  • Work history verification: Must verify at least two most recent employers or last 7 years if only one employer.
